2022 Medication Therapy Management (MTM) Program

Our Medication Therapy Management (MTM) program can help our members manage their medications in special situations.

This program is voluntary and free to eligible members who meet one of the two following criteria:

  1. 1 Be an At-Risk-Beneficiary under the plan’s Drug Management Program, or
  2. 2 Meet all of the following criteria:
    • Have three of more these medical conditions, and
      • Asthma
      • Chronic Heart Failure (CHF)
      • Chronic Obstructive Pulmonary Disease (COPD)
      • Depression
      • Diabetes
      • Dyslipidemia
      • End-Stage Renal Disease (ESRD)
      • Hypertension
      • Osteoporosis
      • Dementia
    • Take eight of more chronic medications, and
    • Have very high medication costs

A team of pharmacists and doctors developed this program to help our members. The program can help our members and their doctors choose the medications that work best to treat their medical conditions. They also help identify possible medication errors and improve adherence to medications.

As part of the program, eligible members will receive a Comprehensive Medication Review (CMR). The provider will go over medications and medical conditions. It will be followed by Targeted Medication Reviews (TMRs). Upon completion of a CMR, the member will receive a Personal Medication List (PML) and Medication Action Plan (MAP) by mail.
